The CDC states that 90% of healthcare costs are from lifestyle driven chronic disease. 4 Corners Holistic Wellness Coaching exists to empower people to practice positive self-esteem and participate in the discovery of their physical, mental, social, and spiritual wellbeing. This journey is a collaborative effort to uncover our client’s best path forward in achieving optimal wellness through key strategies. Our philosophy is to support, validate, and inspire our client’s journey of self-worth towards stress reduction and lifestyle improvement to achieve a more fulfilling life beginning right now.

What is a holistic wellness coach?

A health and wellness coach is an advocate, guide, and a source of inspiration to anyone on a path of self-discovery and betterment. We provide a safe space for a person to dig deep to uncover how they want to shape their life. We provide tools for a person to identify wellness goals and behaviors they wish to change for optimal health. A coach is an ally during the process of behavioral change that leads to a more ideal and satisfactory life. In this partnership, we co-create a personalized path towards the lifestyle changes that are specific and measurable. The plan addresses all aspects of an individual’s life- physical, mental, societal, and spiritual. Working together we move through the process

Process of Coaching

We will begin our relationship with a discovery session and an intake form. This information will assist us in designing a plan to meet your expressed goal. Coaching is a thoughtful process that involves observing behavior and implementing evidence-based solutions to assist in the desired outcome. It is important to develop a relationship, so our process includes:

A free introductory consultation to get to know each other. We will have a 15 min phone conversation to see if there is a connection. It is important that you hire a coach that you feel you can trust, be motivated by, and confident in.

The initial consultation includes an assessment, 120 min zoom/phone/in person conversation, preliminary goal setting/focus

Weekly or monthly sessions describe the frequency client wishes to follow can be scheduled at 50 min single sessions or packages can be discussed.

In between coaching sessions, I am available by email to answer questions, and celebrate a win, a separate time can be scheduled for any question/concern that will require a more detailed exchange of information.

Due to COVID, the client may determine what method feels best so coaching sessions can be in person, by phone, or with a video conferencing platform (zoom).

How many sessions do I need?

This answer will vary from person to person. Hiring a coach means you are committed to investing in making significant changes to improve your longterm health and wellness. These changes will have different timelines depending on the goals, inhibitors, and lifestyle of an individual. We meet each person wherever they are and facilitate growth through providing tools, confidentiality, dependability, reliability, honesty, respect, and open mindedness to all our clients.

4 Corners Health and Wellness Coaches request a minimum commitment of 3-months (6 Sessions) to the coaching process. This will allow the coaching relationship to develop and be a powerful force in your life. Upon completion of the 6 sessions, coaching will convert to a package or per-session agreement. When it is time to end the coaching relationship, we request a two-week (14 days) notice to give us both time to process closure.

How often do sessions happen?

This answer varies depending on an individual’s needs. We prefer to schedule a 50 minute session every two weeks.

Where do these sessions take place?

The jointly scheduled coaching sessions will take place via zoom or by phone. We offer in-person sessions, but due to COVID, we prefer digital sessions to maintain social distance.

Confidentiality

The information exchanged between the coach Pam Stropko and the client is confidential. The Coach- client relationship is not a legally confidential relationship, so the information is not protected by legal privilege. It is understood that what is said here stays here unless expressed written consent is given from the client to share personal information with others. If legally required or if the client is in imminent harm to themselves or others the coach Pam Stropko has the right to converse with the appropriate authorities/medical professionals. The client is aware of Pam Stropko the coach taking notes and charting progress so there is documentation of the coaching process to measure growth. The client may accept/refuse to allow the data collected to be used for research, keeping specific information anonymous.

Expectations

4 Corners Holistic Wellness Coaches provide a safe platform for education and personal growth based on evidence-based coaching practices. The coaching process includes assessments, motivational interviewing, and implementing wellness-industry exercises. During sessions we ask motivating questions, diligently listen to responses, provide direct feedback, encourage self-finding answers, provide realistic expectations for set goals, assist with finding creative solutions, and will do so in a punctual, professional way. The client agrees to the relationship and is responsible for all actions agreed upon as well as follow up and implementing new behavior. The client is solely responsible for all outcomes.

Success Disclaimer

The coach Pam Stropko does not guarantee any specific outcomes/results from our work together in the coaching process. This is a client-led program, and all actions are taken by the client for their own perceived benefit. All outcomes are a direct result of the client’s decision-making process. The coach Pam Stropko is not liable for any actions or inaction taken by the client.

Client Responsibility

The client agrees to implement changes, do the work, and be responsible for any changes they wish to see in their lives. This includes honesty and openness to change, being on time and consistent with the sessions, doing any additional research or homework, taking accountability for their growth, and being an active part of the coaching process. The client is also aware that setbacks occur and are part of growth and will alert Pam Stropko the coach if any serious issues develop.

Cancellation Policy

Pam Stropko the coach may at any time terminate the relationship with the client if they become disruptive, do not follow the agreements written in this contract, or are not ready to take responsibility for the changes needed in their lives without a refund of any outstanding sessions. If the client does not feel a value in the coaching agreement and does not wish to continue the relationship, they may terminate this contract with a written notice.

The client may need to seek out advice from a licensed mental health professional to assist with any issues that arise. The coach Pam Stropko is not a mental health professional, psychotherapist, substance abuse counselor, or a licensed medical health professional and can refer the client to any of these professionals as the need arises.
